I've got a initial cut of the DCBS order form here (right-click and Save As).
This version doesn't have the ComicBloc coupon code in it. DCBS will be posting the official version as soon as they can.

Just to follow up with you about your question on the way the Excel form is laid out:
I talked to Cam this morning when I got to the store and asked about him about it and he basically confirmed one of the scenarios I laid out for you, which was that it was decision he made a decision 3 or 4 years ago to do it that way for the sake of uniformity--particularly for people who don't always get Previews, etc.
He did agree that doing it that way has seemed to match the way Marvel Previews is laid out less and less, he also pointed out that this trend is now continuing in the DC section as well.
I asked him if he had plans to change the way the order form was laid out and he told me it was something that he had already started to look into, but hasn't had a very good opportunity to really sit down with it to make any big changes just yet.
